บทคัดย่อ/คำอธิบาย :

Japanese encephalitis virus (JEV) infection had occurred in Asia. The incidence of the disease has been recognized in the northern part of Thailand and other part of this country. JE vaccines have been recommended to immunize children in the northern part of Thailand. In the near future, JE immunization will be widely used all over country. It was also recommended for travellers who come to the endemic area. The kinetics of antibody responses after Japanese encephalitis (JE) vaccination were detected by hemagglutination inhibition (HI), plaque reduction neutralization (PRNT) and enzyme linked immunosorbent assay (ELISA) IgM, IgG and IgA. The purposes of this study are comparison of HI, PRNT and ELISA IgM, IgG and IgA antibody responses after vaccination or after natural infection. Two doses of JE vaccine (Nakayama strain) were given to 85 medical students aged 18 to 22 years of the third and fourth classes of Faculty of Medicine Siriraj Hospital, Mahidol University. The JE vaccines were given by three different schedules, i.e., group 1 consisted of 26 vaccinees who received vaccines on days O and 7, group 2 were 32 vaccinees receiving vaccines on days O and 14 and group 3 were 27 vaccinees receiving vaccines on days 0 and 30. Blood samples were taken and made these 3 serological techniques after the first vaccination (day 0) and on days 7, 14, 30 and 60. By PRNT test, 74 JE nonimmune vaccinees who had PRNT titers less than 10 were studied. After first vaccination, the seroconversion rates to JEV on day 7 was 13.2% (7/53), on day 14 was 42.9% (18/42) and on day 30 was 21.7% (5/23). After second vaccination on days 7, 14 and 30, the seroconversion rates to JEV on day 14 was 66.7% (12/18), on day 30 was 77.3% (17/22) and on day 60 was 61.1% (11/18) respectively. The PRNT antibody responses increased during 14 to 30 days and decreased on day 60 after vaccination. After 2 doses of JE vaccination, the seroconversion rates and GMT of PRNT antibodies to JEV by different schedules shown no significant difference by one-way analysis of variance. All 11 vaccinees who had preexisting antibodies to JE showed anamnestic responses by PRNT test, increasing of PRNT titers were demonstrated on day 7 and persisted until day 60. By HI test, of the 85 medical students 72, 83.5 and 88.2% had HI antibodies against JE, DEN-1 and DEN-4 antigens compared to 12.9, 77.6 and 83.5% when determined by PRNT. A11 24 students who had negative HI antibodies were nonimmune to JE. The subjects who had PRNT antibodies had HI antibodies at wide range of titers from 10 to 320. Negative HI antibody might reflect negative protective antibody to JEV but positive HI antibody at even high titer might be the cross reactivity. After second vaccination, the seroconversion rates were 50 to 73.9% by HI test which corresponded closely to PRNT test. Cross-reactivities of vaccinees to DEN-1 and DEN-4 viruses were demonstrated on the days which shown high antibody titers, but less than cross-reactivity of patients. ELISA IgM, IgG and IgA could be used for distinguish JE and dengue infections, but were not sensitive and specific enough for measure the specific antibody before and after vaccination.
