บทคัดย่อ/คำอธิบาย :

This descriptive research was designed for 3 objectives. First, to examine the relationship between sekf care agency and quality of life in head & neck cancer patients while receiving and after the completion of radiotherapy while the selected basic conditioning factors of sex, marital status, duration of education, living arrangement and stage of cancer were statistically controlled. Second, to compare the mean scores of self care agency and quality of life between while receiving and after the completion of rediotherapy. Finally, to explore qualitative data pertained to life changes since the diagnosis and radiation treatment, areas of concern and motivation to fight against the disease. Purposive sample of 60 head & neck cancer patients receiving radiotherapy during August 1990-January 1991 from radiotherapy outpatient clinic in Ramathibodi Hospital was obtained for the first data collection and followed to the second data collection with 50 subjects remained. Self care agency was assessed by Self Care Agency Scale developed by the investigator and quality of life was measured by Qualtiy of Life Index for cancer patients developed by padilla & Grant (1985). In addition, qualitative data were obtained through in-depth interview by the investigator. Data were collected twice, during the third or fourth week of radiotherapy and after the completion of radiotherapy for two to four weeks. Results of the study indicated significantly positive relationship between self care agency and quality of life while selected basic conditioning factors were statistically controlled both while receiving and after the completion of radiotherapy (all ps<.001). Further analysis indicated that self care agency and quality of life contributed to each other (p<.001). Mean scores on both self care agency and quality of life while receiving radiotherapy were significantly lower than those of after the completion of radiotherapy (all ps<.01). Furthermore, qualitative data revealed that all patients in this study know about their diagnosis of cancer and the need for radiotherapy. Six areas of concern according to Weisman (1979) were identified. They were health concerns, selfs appraisal concerns, work and financial concerns, family and significant relationships, friends and associated and existential concerns. In spite of these concerns, all of the patients were inspired by the hope and the will to fight against the cancer. The results of this study support validity of Orems theory of Self Care Deficit which proposes the relationship of self care agency and quality of life, implication for practice, education, and further research are proposed.
